MINIMUM EDUCATION AND EXPERIENCE Bachelor’s degree in Physical Therapy with two to three years progressively responsible physical therapy experience. Masters degree in physically therapy and one to two years of home care experience strongly preferred. Current State of Wisconsin Physical Therapy licensure required. Member of American Physical Therapy Association and Wisconsin Physical Therapy Association preferred.
SPECIAL REQUIREMENTS This position requires travel to patient homes and meeting locations. Must maintain a valid Wisconsin driver’s license. Must maintain an acceptable driver’s record per company policy. Must maintain proof of current auto insurance. Must have access to a vehicle in good working condition. Requires evening, weekend, holiday, and on-call responsibilities.
LOSS OF LICENSURE OR CERTIFICATION Any loss of licensure or certification may affect employment status. KNOWLEDGE, SKILLS, AND ABILITIES Knowledge of the principles and practices of physical therapy allowable under professional licensure. Knowledge of American Physical Therapy Association (APTA) code of ethics. Knowledge of Medicare requirements and proper documentation. Knowledge of how to plan and coordinate patient care. Knowledge of current evident based practice (EBP) guidelines. Knowledge and application of physical and psychosocial assessment. Knowledge of medication interaction. Knowledge of audit. Knowledge of Medical Safety Data (MSD). Knowledge of Minimum Data Set (MDS). Knowledge of basic clinical and rehabilitative equipment. Knowledge of outcome based quality improvement (OBQI) and outcome based quality management (OBQM). Ability to maintain, alter or terminate physical therapy based on systematic monitoring of patient’s progress.
Computer Skills: Ability to use a computer for word processing, data entry, and email. Skilled in Microsoft Word, Excel and Outlook. Human Interaction: Ability to exchange, explain, demonstrate and clarify information for others within established polices, procedures and standards. Ability to maintain good working relationships with individuals and families.
Data Utilization: Ability to compile, copy, record and compute data. Ability to reference and apply data.
Equipment Utilization: Ability to operate a variety of standard office equipment including a computer, calculator, copier, fax, scanner and telephone. Ability to operate and maintain basic medical equipment including an oximeter, thermometer, goniometer, and blood pressure cuff. Ability to operate a motor vehicle.
Mathematical Aptitude: Ability to perform basic mathematical skills such as add, subtract, multiply and divide. Ability to calculate percentages, ratios, and averages for when measuring vital signs. |
