Two positions available due to growth. Physical Therapists will work with a talented & experienced rehab team.

Job Summary: Responsible for providing evaluation and treatment of Out-patient physical therapy services with a personalized patient center focus. Assumes responsibility for patient education, collaborates with other professionals and members of the patient care team to ensure optimal patient outcomes and coordinates the delivery of patient care in an efficient and cost effective manner. ELECTRONIC MEDICAL RECORDS, SIGN-ON-BONUS, RELOCATION ASSISTANCE AND EXCELLENT MENTORING PROGRAM AVAILABLE

Job Requirements: Minimum Bachelors degree and licensed in the State of Wisconsin or eligible. If eligible, must obtain license within 3 months of hire or after two attempts to pass test.
