Location: Weston, WI

Responsible for screening, consulting, evaluating and treating patients in short term rehab, long term rehab, home health, and outpatient. Patient/family education, staff training, wellness at our assisted living centers.

Work with a interdisciplinary team that consists of physical, occupational, speech therapists. Also work closely with nursing and activities.

Attend orthopedic and neurological appointments with patients as needed. Documentation requirements are evaluations, weekly and daily summaries plus monthly and discharge summaries. Supervision of PTA's.
