Provides comprehensive physical therapy services to patients by assessing, planning, implementing, and documenting physical therapy services. Demonstrates advanced evaluation and treatment skills and involved in program development and staff education. Current license by the State of Wisconsin. Bachelor's of Science degree in Physical Therapy or entry level Masters degree in Physical Therapy required. At least two years clinical experience. Basic Life Support Certification. Valid driver's license.
