Physical Therapist Needed – Join Our Compassionate Team!

Retention bonus of $10,000 payable in quarterly installments during your first year of employment.

At HCS, we are dedicated to providing exceptional, compassionate home care to individuals who are recovering from surgery, managing chronic conditions, or improving mobility. We believe that every patient deserves high-quality care in the comfort of their own home. We are seeking a skilled and compassionate Home Care Physical Therapist to join our team and help our patients achieve their rehabilitation goals. This position is full time with hourly pay.

Responsibilities:

Evaluate patients in their home environment to assess physical mobility, strength, and function.
Develop and implement individualized treatment plans tailored to the needs of each patient.
Provide hands-on physical therapy to improve mobility, reduce pain, and enhance daily living skills.
Collaborate with interdisciplinary team members including nurses, occupational therapists, and physicians to ensure comprehensive patient care.
Educate patients and their families on exercises, techniques, and self-care strategies for optimal recovery.
Document progress and adjust treatment plans as necessary based on patient response and goals.
Monitor and ensure patient safety during therapy sessions.

Requirements

Graduate of a program in physical therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association or its equivalent;
One year of professional physical therapy experience under the appropriate supervision; or
A combination of education and experience that demonstrates the person has acquired the necessary competencies required for the position.
Possession of a current State of New Hampshire license to practice as a registered physical therapist.
Must be able and willing to learn to utilize a portable electronic field device. Must be able to manage electronic patient health information (ePHI) remote access in accordance with HCS policies and procedures.
Valid driver’s license and access to an automobile during working hours that meet the State of New Hampshire registration regulations and auto liability insurance.

Home Healthcare, Hospice and Community Services (HCS) is a non-profit organization providing comprehensive services in southwestern New Hampshire.
