Interim Healthcare of Scranton is hiring a Home Health Physical Therapist (PT) for adult patients in Wayne & Pike Counties. This is a W2 role with a realistic daily schedule and 1:1 patient care.

Mon–Friday schedule (no weekends)
4–6 patients/day average
Adult-only home health
Support from an experienced care team (RN/LPN/CNA/HHA/OT/SLP)

Responsibilities

Provide in-home PT evaluation and treatment
Create and update individualized plans of care
Document patient progress and outcomes
Teach patients and caregivers exercises + safety strategies
Coordinate care with the interdisciplinary team

Requirements

Active PA Physical Therapist License
Reliable transportation + valid driver’s license
CPR certification
