Opportunities to work across multiple senior care settings, including in-home supportYour Role as a Physical Therapist:
-
Conduct initial and ongoing evaluations of residents’ strength, mobility, balance, and endurance
-
Develop and implement individualized treatment plans to restore function and improve independence
-
Assist residents recovering from injuries, surgeries, or illnesses with targeted therapy
-
Support activities of daily living including walking, transferring, and dressing
-
Deliver strengthening, flexibility, and coordination exercises to improve functional mobility and prevent injury

Educate residents, families, and staff on fall prevention, mobility strategies, and safe use of assistive devices
-
Collaborate with a multidisciplinary team of nurses, OTs, SLPs, and other healthcare professionals

Participate in care planning meetings and maintain accurate documentation of all therapy services

Qualifications
- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
