RVNAhealth is seeking a licensed and experienced Physical Therapist to join our In-Home Outpatient (IHOP) team.
This full-time position provides outpatient-level physical therapy services in patients' homes and assisted living facilities across Central and Southern Fairfield County (Wilton, Weston, Redding, Norwalk, Westport, New Canaan, etc.)
You will work primarily with post-surgical patients, including those recovering from total joint replacements. Your responsibilities will include evaluations, treatment, documentation, and collaboration with physicians and care teams to help patients regain mobility and remain safely at home.
Key Responsibilities

Conduct comprehensive evaluations and develop individualized care plans.

Deliver physical therapy treatments in accordance with Connecticut PT practice guidelines.

Coordinate with physicians, families, and care teams to ensure continuity of care.

Refer patients to additional home health services when needed.

Participate in case conferences and interdisciplinary team meetings.

Provide patient and caregiver education on treatment plans and home exercises.

Perform pre-operative safety evaluations and post-surgical follow-up visits.

Maintain accurate, timely clinical documentation and obtain physician orders as required.
Qualifications

Bachelor's degree or higher from an accredited Physical Therapy program.

Current license to practice Physical Therapy in the state of Connecticut.

Minimum of 2 years of clinical experience, preferably in home health or outpatient rehab.

Familiarity with Medicare Part B regulations and billing practices for in-home services.

Strong communication and organizational skills.

Experience working in assisted living facilities is a plus.

Compensation:
Our target pay range for this position is $90,000 to $100,000 per year. This pay range represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
