Your Role:
-
Conduct initial and ongoing evaluations of residents’ strength, mobility, balance, and endurance
-
Develop and implement individualized treatment plans to improve functional independence
-
Support residents recovering from injuries, surgeries, or illness through targeted therapy
-
Provide interventions to enhance activities of daily living including walking, transferring, and self-care
-
Educate residents, families, and staff on safe mobility, fall prevention, and adaptive equipment
-
Collaborate with multidisciplinary teams to ensure holistic care and optimal outcomes
-
Maintain accurate documentation for assessments, treatments, and progress

Qualifications
- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
