About the Role:
As a Physical Therapist in a Skilled Nursing Facility (SNF) on a PRN basis, you will play a critical role in the rehabilitation and recovery of patients with a variety of physical impairments and disabilities. Your primary objective will be to assess, plan, and implement individualized treatment programs that enhance patients' mobility, reduce pain, and improve overall functional independence. You will collaborate closely with interdisciplinary healthcare teams, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care. This role requires adaptability to varying patient needs and the ability to provide high-quality therapy services on an as-needed schedule. Ultimately, your work will contribute significantly to patients' quality of life and successful transitions back to their homes or other care settings.
As a Physical Therapist, you will:
Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
Applies concepts of infection control/universal precautions in performing patient care
Assure continuity of quality patient care delivered with appropriate documentation
Provide effective communication to patient/family, team members and other health care professionals
Promote agency philosophy and administrative policies
Maintain and complete all documentation within company stated guidelines.
Requirements
Successful completion of accredited physical therapy program and state licensure exam.
Valid Physical Therapy license
1 year of home health experience preferred
CPR certification
Valid Driver’s license
Valid Auto Insurance
HomeCare HomeBase experience preferred
