Family Tree Rehabilitation, part of The Campus of Anna Maria, is seeking a Physical Therapist (PT) to support our therapy team as needed. Situated on a beautiful wooded campus in Ohio, we are a family-owned and operated skilled nursing facility dedicated to providing high-quality, resident-centered rehabilitation services in a collaborative and supportive environment.
Position Summary
The PRN Physical Therapist is responsible for evaluating residents, developing and implementing individualized physical therapy plans of care, and helping residents improve mobility, strength, balance, and overall functional independence.

Key Responsibilities of Physical Therapist (PT)
Perform physical therapy evaluations and establish individualized plans of care
Provide skilled physical therapy treatment services in accordance with physician orders
Collaborate with and supervise PTAs as applicable
Accurately document evaluations, treatments, progress notes, and discharge planning
Participate in interdisciplinary care planning and team communication
Ensure compliance with facility policies, professional standards, and regulatory requirements

QualificationsQualifications for Physical Therapist (PT)
Current Ohio Physical Therapist (PT) license in good standing
Graduate of an accredited Physical Therapy program
Experience in skilled nursing or rehabilitation setting preferred
Strong clinical judgment, communication, and time-management skills
Compassionate, resident-centered approach to care
