About the Role:
The Physical Therapist PRN plays a critical role in delivering high-quality rehabilitative care to patients within their homes, focusing on improving mobility, reducing pain, and enhancing overall physical function. This position requires the ability to assess patient needs accurately, develop personalized treatment plans, and implement therapeutic interventions that promote recovery and independence. The therapist will collaborate closely with patients, families, and interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes. Flexibility and adaptability are essential, as the PRN role involves working on an as-needed basis to meet fluctuating patient demands across diverse home environments. Ultimately, this role contributes significantly to patients' quality of life by enabling them to regain physical capabilities and maintain their independence in a comfortable, familiar setting.
As a Physical Therapist, you will:
Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
Plan, prepare, or implement individually designed physical treatment programs to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
Applies concepts of infection control/universal precautions in performing patient care
Assure continuity of quality patient care delivered with appropriate documentation
Provide effective communication to patient/family, team members and other health care professionals
Promote agency philosophy and administrative policies
Maintain and complete all documentation within company stated guidelines.
Requirements
Successful completion of accredited physical therapy program and state licensure exam.
Valid Physical Therapy license
1 year of home health experience preferred
CPR certification
Valid Driver’s license
Valid Auto Insurance
HomeCare HomeBase experience preferred
