PIVOT Rehabilitation is hiring a Licensed Home Health Physical Therapist (PT) for a Full-Time home health physical therapist job providing in-home physical therapy services to adult and senior patients in Dallas, TX and surrounding DFW areas.

Job Responsibilities
Perform home health physical therapy evaluations, including OASIS assessments
Develop and update individualized Plans of Care (POC)
Provide skilled home health physical therapy for orthopedic, neurological, and neuromuscular conditions
Deliver interventions focused on mobility, balance, strength, gait training, and fall prevention
Educate patients and caregivers on safe transfers, home exercise programs, and use of durable medical equipment
Complete accurate and timely documentation in the EMR
Communicate with physicians and interdisciplinary care teams
Supervise PTAs and home health aides, as applicable
Follow all home health regulations, safety standards, and infection control policies

QualificationsQualifications
Bachelor’s, Master’s, or Doctorate in Physical Therapy from an APTA-accredited program
Active Physical Therapist (PT) license in the state of practice or PT Licensure Compact eligibility
Home health or rehab experience (1+ year required)
Current CPR certification
Strong communication, clinical judgment, and time-management skills
Knowledge of OASIS and home health regulations preferred
Valid driver’s license, reliable transportation, and current auto insurance
