The Physical Therapist is responsible for delivering comprehensive rehabilitative care aimed at restoring function, improving mobility, relieving pain, and preventing or minimizing permanent physical disabilities in patients affected by injury or disease. This role also promotes overall fitness and health through individualized treatment plans and patient education.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Patient Care
Provide treatment to patients that is appropriate for their age, cultural background, and medical condition within assigned clinical areas.
Perform and document an initial examination, evaluating data to identify problems and determine a diagnosis prior to intervention.
Identify patient care needs; communicate findings to the patient and care team; recommend treatment options and courses of action; and implement physical therapy directives.
Plan, prepare, and carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, and prevent physical dysfunction.
Administer therapeutic procedures, including:
Manual therapy, joint mobilizations, decompression and/or traction techniques to relieve pain, optimize mobility, and prevent progression of functional limitations.
Instruction, encouragement, and assistance with non-manual exercises, ambulatory functional activities, and daily living activities.
Guidance in using supportive devices such as crutches, canes, and prostheses.
Instruct patients and families on treatment procedures to be continued at home.
Evaluate effects of treatment at various stages and adjust plans to achieve maximum benefit.
Test and measure patient strength, motor development, sensory perception, functional capacity, and respiratory/circulatory efficiency; record data accurately.
Documentation & Compliance
Document patient treatment, progress, prognosis, and response in the medical record or electronic system in accordance with peer review standards.
Identify and document goals, anticipated progress, and plans for reevaluation.
Maintain compliance with legal, accreditation, and organizational standards by adhering to federal, state, and local regulations, as well as PartnerCare policies.
Collaboration & Communication
Review physician referrals and patient medical records to determine diagnosis and required physical therapy treatment.
Confer with patients, medical practitioners, and other appropriate team members to plan, implement, and assess intervention programs.
Provide clear education to patients and families regarding conditions, treatment plans, and expected outcomes.
Compliance, Safety & Quality
Ensure compliance with HIPAA, OSHA, infection control standards, and clinic safety protocols
Maintain all required clinic logs (medication, lock box, autoclave, crash cart, safety, nursing observation, temperature logs, etc.)
Oversee vaccination and clinical administration protocols (e.g., flu shots, TB testing, hepatitis vaccines, sinus cocktails) as applicable
Maintain a safe and clean working environment by following established procedures, rules, and infection-control protocols.
Technology & Systems
Coordinate with IT on office equipment, systems, and technology needs
Ensure proper use and maintenance of EMR systems and clinic technology
Patient Experience
Handle patient inquiries, concerns, and complaints in a professional and timely manner
Promote high levels of customer service, patient safety, and patient education
PROFESSIONAL QUALIFICATIONS:
Education & Licensing Requirements:
Masters / Doctoral Degree in Physical Therapy from an accredited university.
Current active and unrestricted (without sanctions) State License.
Basic Life Support/Advanced Cardiac Life Support certification.
Experienced preferred, but new graduates are welcome to apply!

QualificationsEducation and Experience:
Associates Degree in Physical Therapy from an accredited university or technical institution
Current physical therapist license
At least three years of experience as a physical therapist
Knowledge of legal, medical, and accreditation compliance
Alertness and careful attention to detail is required to avoid injury. Must be able to deal with stressful and/or complex situations
Current physical therapist license
Physical Requirements:
Must be able to physically work with the patient to ensure exercises are performed correctly
