Position Overview:
The Physical Therapist will be responsible for evaluating participants and administering tailored physical therapy treatments while monitoring their progress. Additionally, the Physical Therapist will conduct assessments, develop care strategies, and coordinating care delivery while maintaining communication with the team.
Essential Job Functions:
Evaluates participants and administers physical therapy treatments.
Records progress and instructs participants and/or family in care.
Supervises work activity of restorative aides, assistants, and students.
Will work in the PACE Center and in participant homes.
Conducts initial assessment and period reassessments of participants.
Develops plan of care for participants.
Coordination of 24-hour care delivery.
Regularly informs the interdisciplinary team (IDT) of the medical, functional, and psychosocial condition of each participant.
Remains alert to pertinent input from other team members, participants’ caregivers.
Documents change in participant’s medical record consistent with documentation policies.
Evaluates participant needs for durable medical equipment (DME), prescribes such equipment, and monitors its appropriate use and ongoing condition to support participant mobility and rehabilitation goals.
Qualifications and Experience:
Must be at least 18-years of age
Must have a doctorate degree in physical therapy from an accredited university. Exception is made for applicants who graduated prior to 01/01/2003 and possess a bachelor’s degree and license in physical therapy
Must hold a Physical Therapist license in Kansas
Must have working knowledge of Microsoft Office Suite.
Prefer a minimum of 2 years working as a Physical Therapist
Prefer a minimum of 1 year working with the frail or elderly
Physical Requirements:
Medium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly.
