Responsibilities

What you’ll do

Evaluate patients and develop individualized physical therapy treatment plans
Provide skilled therapy services in patients’ homes
Re-evaluate progress and adjust care plans as needed
Collaborate with interdisciplinary care team members
Educate patients and families on treatment goals and techniques
Complete clinical documentation in accordance with agency standards

Compensation & support

Hourly pay range: $39.25 – $60.84 (based on experience)
6-month housing bonus (conditions apply)
Relocation bonus up to $10,000 (restrictions apply)

Qualifications

REQUIRED QUALIFICATIONS

Must be a licensed Physical Therapist in Vermont and New Hampshire or hold a compact license.
Graduate of an accredited school in Physical Therapy

PREFERRED QUALIFICATIONS

One year of experience as a PT in a home health care setting is preferred

Required Licensure/Certifications

Unencumbered PT license in VT and NH or compact.
