In this role, your exceptional communication skills will be key to your success. You will schedule independent home visits to screen and treat your patients and continually reevaluate and document their progress. Qualifications

Minimum of Bachelor of Science Degree in Physical Therapy from accredited institute of learning
Licensed and registered as a PT in New York State
Minimum of 1 year experience in a health care setting; Home Care experience highly preferred
Must be able to provide own transportation in order to make visits in assigned area(s); vehicle/drivers license may be required
