What You’ll Do:

Deliver exceptional PT services to evaluate and treat patients in their homes
Work closely with a compassionate care team
Help patients regain independence and quality of life

Your Role:

Evaluate and treat patients in the home
Help improve mobility, strength, and safety
Document using EMR (Kantime a plus)

Qualifications

What You’ll Need:

Active CO PT license
1+ year in acute care or home health (required)
Medicare home health experience is highly preferred
Passion for patient-centered care
EMR experience
