Physical therapist position details:

We are looking for a per diem (as needed) physical therapist to provide coverage for vacations, leave time, and sick time. Applicants would need availability Monday – Friday, day shift.

Physical therapist job duties include:

Performing skilled therapy assessments and evaluations.
Developing and implementing individualized treatment plans.
Administering a range of physical therapy techniques and modalities.
Monitoring patient progress and adjusting treatment plans as needed.
Maintaining accurate and timely documentation in accordance with progression and regulatory standards.
Collaborating with interdisciplinary team to ensure coordinated care.

Qualifications

Physical therapist qualifications:

Current and valid NYS license to practice as a physical therapist is required.
Must have the ability to work independently and as part of a team.
Must be able to communicate well verbally and in writing.
Proficiency with electronic medical record (EMR) system is required.
Experience in subacute rehab or outpatient geriatric settings is preferred.
