Description

We are currently seeking a Home Health Physical Therapist (PT) to join our growing team. We are hiring for PRN positions. PRN therapists will be paid per visit. As a Physical Therapist, you will provide skilled therapy services in the home setting, focusing on helping patients restore function, improve mobility, and achieve the highest level of independence possible.

Key Responsibilities:

Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
Deliver skilled therapy interventions in the home environment.
Instruct patients and caregivers on therapeutic exercises, mobility, safety, and use of assistive devices.
Monitor patient progress, reassess needs, and update care plans as needed.
Communicate and collaborate with physicians, nurses, and the interdisciplinary care team.
Document care provided in a timely and accurate manner.
Travel to patient homes throughout Cincinnati and surrounding areas.
Maintain flexibility with scheduling during daytime hours, Monday through Friday.

Requirements

Current, active Physical Therapist license in the State of Ohio (required).
1 year of experience in home health or a healthcare facility preferred.
Current CPR certification (required).
Valid driver’s license, reliable transportation, and current auto insurance.
Strong clinical assessment, organizational, and communication skills.
Compassionate, patient-centered approach to care.
Ability to work independently and manage a flexible caseload and schedule.
