Key Responsibilities

Develop and update individualized care plans
Provide in-home physical therapy services per physician’s plan of care
Assess functional status, mobility, and home environment
Educate patients and families on therapy techniques and equipment
Maintain accurate documentation and communicate progress to physicians

Requirements

Licensed Physical Therapist in the State of Arizona
Degree from a CAPTE-accredited program; passed state-approved exam
Minimum 2 years’ experience (home health preferred)
Current CPR certification
Fingerprint Clearance Card (or immediate ability to apply)
Valid driver’s license, insurance, and reliable transportation
