Physical Therapist (PT) – Home Health

Full-Time Baylor | Friday–Monday

We're currently seeking a licensed Physical Therapist to join our growing Home Health Team.

What You'll Do

As a Home Health Physical Therapist, you'll help patients regain independence, improve mobility, and safely recover in the comfort of their own homes by:

Evaluating patients and developing individualized treatment plans based on physician orders.
Providing skilled physical therapy services to improve strength, mobility, balance, endurance, and functional independence.
Monitoring patient progress and modifying treatment plans as appropriate.
Communicating changes in patient condition to the physician and interdisciplinary care team.
Educating patients and caregivers on home exercise programs, safety techniques, adaptive equipment, and fall prevention.
Supervising Physical Therapist Assistants (PTAs) in collaboration with the Director of Rehabilitation.
Completing accurate and timely clinical documentation.
Participating in case conferences, care coordination, quality improvement activities, and continuing education.

Qualifications

Graduate of an accredited Physical Therapy program.
Current Florida Physical Therapist license in good standing.
Minimum one (1) year of physical therapy experience required.
Home health experience preferred but not required.
Current CPR certification.
Valid Florida driver's license, reliable transportation, and proof of auto insurance.
Strong communication, time management, and patient education skills.
