Physical Therapist Responsibilities

As a Home Health Physical Therapist, you will:

Evaluate patients in their home environment
Develop and implement individualized physical therapy treatment plans
Provide skilled physical therapy interventions based on patient needs
Collaborate with physicians, nurses, therapists, and other members of the care team
Monitor patient progress and adjust treatment plans as appropriate
Educate patients and caregivers on treatment plans, exercises, and strategies to support recovery
Maintain timely and accurate clinical documentation
Manage your daily schedule and patient visits independently

Qualifications

We’re looking for compassionate, organized, and patient-focused Physical Therapists who have:

Bachelor’s or Master’s degree in Physical Therapy from an accredited college or university
Current Physical Therapist (PT) license in the state where you will practice
Minimum of one year of healthcare experience
Current valid driver’s license and reliable ability to travel between patient homes
Ability to spend approximately 20% of the workday driving to and from patient locations
Strong time-management and organizational skills
Ability to consistently meet critical documentation and charting deadlines
Comfort using tablets, smartphones, and EMR/EHR software
Ability to work independently while collaborating effectively with a multidisciplinary care team
Compassionate approach to patient care and a commitment to improving patient outcomes

Schedule & Travel

Schedule: Monday–Friday, 8:00 AM–6:00 PM
Weekend: 1 weekend per month
Holidays: 2 holidays per year
Travel: Local travel to patients' residential homes
