Responsibilities

Implementation of physical therapy treatment plans of care.
Coordination of care with entire care team
Educating patients and family members
Preparing timely, complete and accurate clinical notes
Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members

Qualifications

Physical Therapist/DPT licensed in the State of Texas
Skilled Nursing and Long Term Care
Experience with EMR (PCC and NetHealth) is preferred
Cares for people and will go above and beyond to provide outstanding patient care
Scheduling flexibility that allows you to do your best work while enjoying great quality of life
Demonstrates a dedication for outstanding delivery of patient care and teaching
