About the Role
As a Physical Therapist in Union Grove, you will help residents improve mobility, regain function, and maintain independence. You’ll develop individualized treatment plans, collaborate with a skilled interdisciplinary team, and deliver meaningful care that changes lives.
Key Responsibilities
- Conduct comprehensive assessments of residents’ physical function and mobility
- Develop and implement individualized treatment plans
- Provide skilled physical therapy interventions
- Educate patients and caregivers on mobility strategies and fall prevention
- Recommend appropriate assistive devices and modifications
- Maintain accurate, timely clinical documentation
- Collaborate with nursing and therapy colleagues to ensure holistic care
Qualifications
- Graduate of an accredited program with a bachelor’s, master’s, or doctoral degree in Physical Therapy
- Licensed or eligible for licensure in the State of Wisconsin
- Strong clinical, communication, and organizational skills
- A passion for working with older adults in a skilled nursing or rehabilitation setting

Qualifications
- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
