As a Physical Therapist, You Will:
- Perform initial and ongoing evaluations to assess mobility, strength, balance, and endurance
- Develop and implement individualized treatment plans focused on restoring function and independence
- Deliver hands-on interventions to help residents recover from surgery, illness, or injury
- Improve quality of life by enhancing functional movement and reducing pain
- Educate residents, families, and staff on mobility strategies and fall prevention
- Recommend and train in the use of assistive devices (walkers, braces, wheelchairs, etc.)
- Collaborate with a multidisciplinary care team to ensure holistic, person-centered care
- Document thoroughly to meet healthcare regulations and ensure continuity of care
This role supports residents in skilled nursing, assisted living, and home health settings—making a real difference where it's needed most.

- Active Wisconsin Physical Therapist license
- Strong clinical skills, critical thinking, and a team-first mindset

Qualifications
- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
