Your Responsibilities
- Provide compassionate and effective physical therapy services to adult and geriatric residents
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence
- Support residents in achieving personal goals and rehabilitation milestones
- Accurately document care and maintain compliance standards (experience with Net Health a plus)
- Collaborate with therapy and nursing teams to ensure patient-centered outcomes
- Educate residents and families on safe techniques and strategies to maximize independence
- Participate in team meetings and contribute to care planning
What You Bring
- Current Wisconsin PT license
- Strong clinical skills and a passion for working with older adults

Qualifications
- Graduate of an accredited school with a bachelors, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
