Position: Physical Therapist (PT)

H2 Health is seeking a dedicated and enthusiastic Physical Therapist to join our team. Our clinics prioritize providing high-quality, patient-centered care in a supportive, collaborative environment. With our focus on personalized care, you will be able to develop individualized treatment plans, assess functional abilities, and implement therapeutic interventions that improve patients' quality of life.
Your Responsibilities:
- Conduct thorough evaluations of patients' physical abilities and limitations.
- Collaborate with healthcare professionals to develop comprehensive treatment plans.
- Provide one-on-one therapy sessions to address rehabilitation needs.
- Document patient progress and adapt treatment plans as necessary.
- Educate patients and their families on exercises, recovery techniques, and injury prevention.
Requirements
- Must hold a Physical Therapy degree from an accredited program.
- Active state licensure or eligibility for licensure as a Physical Therapist.
- Strong interpersonal communication skills and a commitment to patient-centered care.
- Experience in an outpatient setting is preferred, but new graduates are encouraged to apply.
