About the Role
As a Physical Therapist at our Union Grove campus, you’ll:
-
Conduct comprehensive assessments of residents’ mobility and physical function
-
Develop and implement individualized treatment plans
-
Deliver skilled PT interventions to improve quality of life
-
Educate residents and caregivers on safe mobility and fall prevention
-
Recommend appropriate assistive devices and environmental modifications
-
Collaborate with nursing and therapy teams for holistic, person-centered care
-
Maintain accurate and timely clinical documentation
Qualifications
-
Graduate of an accredited PT program (Bachelor’s, Master’s, or Doctorate)
-
Licensed or eligible for licensure in Wisconsin
-
Strong clinical, organizational, and communication skills
-
Passion for working with older adults in skilled nursing or rehabilitation settings

Qualifications
- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
