As a Physical Therapist, you will play a critical role in helping patients recover from injuries, manage chronic conditions, and improve their overall physical function and mobility. You will assess patients' physical abilities, develop personalized treatment plans, and implement therapeutic interventions to promote healing and prevent further injury. Your work will involve collaborating with healthcare professionals to ensure comprehensive care and support for patients across various age groups and conditions. The ultimate goal of this role is to enhance patients' quality of life by restoring movement and reducing pain through evidence-based physical therapy techniques. You will also educate patients and their families on exercises and lifestyle modifications to support long-term health and rehabilitation outcomes.

Minimum Qualifications:

As a Physical Therapist, you will:

Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
Applies concepts of infection control/universal precautions in performing patient care
Assure continuity of quality patient care delivered with appropriate documentation
Provide effective communication to patient/family, team members and other health care professionals
Promote agency philosophy and administrative policies
Maintain and complete all documentation within company stated guidelines.

Requirements

Successful completion of accredited physical therapy program and state licensure exam.
Valid Physical Therapy license
1 year of home health experience preferred
CPR certification
Valid Driver’s license
Valid Auto Insurance
HomeCare HomeBase experience preferred
