Greenfield Rehabilitation Agency is expanding and seeking passionate Physical Therapists (PTs) to join our exceptional team at our senior campuses in Lancaster, while also supporting our communities in Prairie du Chien. Physical Therapist Responsibilities:

Conduct initial and ongoing evaluations of residents’ mobility, strength, balance, and endurance.

Identify barriers to functional independence and design individualized physical therapy programs.
-
Address challenges related to mobility, posture, gait, and injury or illness recovery.

Provide interventions to help residents regain strength, prevent falls, and maintain independence.

Support residents in daily functional tasks, recommending adaptive equipment as needed.

Conduct exercises and mobility programs to enhance coordination, flexibility, and overall physical function.

Assess living spaces to recommend modifications for safety and accessibility.

Educate residents, families, and staff on therapy goals, safe mobility, and fall prevention.

Collaborate with OTs, SLPs, nursing, and interdisciplinary teams to ensure holistic care.

Maintain accurate records of assessments, treatments, and progress, ensuring compliance with healthcare regulations and community policies.

Qualifications
- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
