Position Summary:

As a Mobile Physical Therapist with Home Health Care, Inc., you'll bring expert therapy directly to patients—empowering them to regain confidence, strength, and independence where they feel most at ease. This is therapy with purpose, flexibility, and the space to make meaningful clinical decisions.

Essential Duties and Responsibilities

Perform in-home mobility and functional assessments.
Develop and execute individualized rehabilitation plans.
Provide hands-on therapy, strengthening exercises, and mobility training.
Recommend home safety modifications and adaptive equipment.
Communicate with nurses, caregivers, and the care team to ensure coordinated care.

Knowledge, Skills and Abilities

Passion for individualized, patient-centered therapy.

Education & Experience

Active Minnesota Physical Therapist license.
Valid driver's license and reliable transportation.
Home health experience preferred, but outpatient and inpatient PTs strongly encouraged to apply.
