Your Role as a Physical Therapist
As a Physical Therapist with Concierge Home Care, you will deliver personalized, patient-centered care while leveraging advanced technology and AI-supported tools designed to streamline documentation and improve efficiency.
Key Responsibilities:
- Travel to patients’ homes to provide individualized physical therapy services that promote recovery, mobility, and independence
- Conduct comprehensive evaluations to assess patient conditions and rehabilitation needs
- Develop and implement goal-oriented treatment plans in collaboration with patients, caregivers, and the interdisciplinary care team
- Utilize AI-assisted documentation and charting tools to support timely, accurate note completion and faster turnaround times
- Guide and supervise Physical Therapy Assistants to ensure high-quality, compliant care delivery

Qualifications
- Unencumbered Florida Physical Therapy License (required)
- Valid driver’s license, auto insurance, and reliable transportation (required)
- Experience with OASIS documentation (preferred)
- CPR Certification (required)
- Ability to delegate and supervise Physical Therapy Assistants (preferred)
