Role Responsibilities
-
Lead and oversee daily rehabilitation operations, ensuring high-quality, patient-centered therapy services

Provide direct patient care as needed while maintaining program oversight

Collaborate with the interdisciplinary team to develop and implement individualized treatment plans

Mentor, coach, and develop therapy staff to promote professional growth and engagement

Oversee documentation, scheduling, productivity, quality assurance, and regulatory compliance
-
Participate in Medicare meetings and coordinate care with residents, families, and facility leadership

Drive evidence-based practice, innovation, and continuous program improvement
Qualifications
-
Licensed Physical Therapist (PT) in the state of Wisconsin
-
Prior leadership or supervisory experience preferred

Experience in skilled nursing or senior living settings required

Strong organizational, communication, and problem-solving skills

Passion for working with older adults and leading high-performing therapy teams
