Job Summary
As a Physical Therapist (PT), you'll evaluate patients, create and carry out treatment plans, and coordinate with our rehab team to provide top-tier care in the home setting.
Key Responsibilities
Evaluate patient’s condition and functional level
Develop and implement treatment plans
Supervise PTAs and HHAs per plan of care
Provide in-home therapy and adaptive equipment guidance
Maintain accurate and timely documentation
Ensure patient and family education for home programs
Submit notes/visits in a timely manner
Adhere to HIPAA confidentiality standards
Qualifications Requirements
Active PT license in Ohio
1+ year of PT experience (preferred)
OASIS and WellSky/Kinser experience (strongly preferred)
Physical exam (within 6 months of start)
Current TB test and CPR certification
Valid driver’s license and auto insurance
Ability to attend orientation in Dublin, OH
