The Lutheran Home is seeking a Licensed Physical Therapist to join our team on a PRN weekend basis. This role focuses on providing individualized rehabilitation services to support resident mobility, function, and independence. Responsibilities may include, but are not limited to:
- Plan, prepare, and carry out individually designed physical therapy programs to maintain, improve, or restore physical function, alleviate pain, or prevent physical dysfunction
- Perform and document initial exams, evaluating data to identify problems and determine diagnoses prior to intervention
- Evaluate treatment effects and adjust treatments to achieve maximum benefit
- Identify and document goals, anticipated progress, and re-evaluation plans in a timely manner
- Review referrals and medical records to help determine required physical therapy treatment
- Test and measure patient strength, motor function, sensory perception, functional capacity, or respiratory/circulatory efficiency and record data
- Perform all responsibilities following safety and infection control procedures
- Follow CMS, insurance, and AMA guidelines for skilled services, documentation, and billing
- Adhere to the Physical Therapy Statutes established by the Missouri Board of Healing Arts
In this PRN weekend position, we are seeking availability on weekends as needed, with flexible hours between 8:00 am and 4:30 pm.
License/Certification required:- Licensed to work as a Physical Therapist in Missouri
- Experience in a skilled nursing facility preferred
