Responsible for providing Physical Therapy (PT) service and is responsible for maintaining standards of professional and ethical practice, of both themselves and all assigned physical therapy assistance, in the provision of these services. Physical therapist will provide service to pediatric thru geriatrics in the settings of inpatient hospital, outpatient hospital, and the schools
Requirements
Knowledge & Experience
Knowledge-
- Orthopedic/Work Injury Management knowledge base.
- Must have in-depth knowledge of principles, literature, standard practices, methods, and new developments in the area of Physical Therapy.
- Knowledge of Work Injury Management systems and philosophy of care.
Education-
- Graduate of a School of Physical Therapy approved by the American Physical Therapy Association.
Experience-
- Currently licensed or eligible for licensure in Indiana.

ACCOUNTABILITY
- Reports to the Rehabilitation Director.
QUALITY ASSURANCE-KEY RESPONSIBILITIES
- Complete evaluations to develop a plan of care including frequency/duration, short term goals, long term goals, home exercise program, pain management program, and discharge planning in a timely manner according to the departmental documentation policy.
- Accepts responsibility for patient’s personal care and environment throughout the treatment.
- Follows established procedure and observes safety precautions in the application of modalities.
- Carries out positioning and exercise plan of care.
- Observes and reports the patient’s responses
- Acts as an assistant when other therapist are performing tests, and complex treatment procedures.
- Carries out responsibilities appropriate to the established physical therapy service.
- Participates in clerical and reception activities.
- Complies with procedures for maintenance of supplies and equipment and carries out duties necessary to comply with the related needs as delegated to him/her.
- Maintains surveillance of environmental conditions within the therapy service.
- Complete appropriate documentation in a timely manner in accordance with departmental policies related to initial evaluation, re-evaluations (as needed), provider communication, patient education, daily notes, progress notes, and discharge summary
- Provide services within the Physical therapy scope of practice, in accordance with State Practices laws, to all demographics presenting to Rush Memorial inpatient, outpatient, and school settings.
PUBLIC RELATIONS
- Maintains effective channels of communication for the PT and OT with other therapy staff, physicians, nursing staff, and other hospital staff.
- Takes appropriate action in problem situations to offset adverse effects.
- Projects a professional and positive attitude regarding the hospital.
LEADERSHIP
OTHER RESPONSIBILITIES
- Seeks opportunities for self development and identifies areas of improvement.
- Conducts self in professional manner acting as a role model while practicing and promoting effective guest relations.
- Observes Hospital dress guidelines and wears ID Badge.
- Provides proper notification for absence and tardiness and is punctual for start of scheduled time.
- Participates in emergency procedures, fire drills, evacuation procedures and internal/external disaster plans and procedures in accordance with departmental responsibilities.
- Maintains equipment assigned to department. Reports malfunctions and orders service as necessary. Maintains department cleanliness procedures.
- Complies with hospital policies and procedures.
- Completes projects and assignments on a timely basis.
- Consistently participates in departmental meetings. Participates in facility meetings as assigned

- Participates in the Continuous Quality Improvement Program under the direction of the Coordinator to meet stated goals and objectives of the Continuous Quality Improvement Program by:
- Assists in data collection.
