What You’ll Do
Deliver in-home physical therapy to patients recovering from illness or injury
Collaborate with a multidisciplinary team (RN, OT, SLP, CNA, etc.)
Assess mobility, document progress, and adjust care plans
Guide patients through exercises and hands-on therapy
Educate patients and families on goals, exercises, and safety
What You’ll Need
Degree from an accredited PT program + active Wisconsin PT license
1+ year of PT experience (home health preferred)
CPR certification
Familiarity with home health regulations
Strong clinical judgment, empathy, and communication skills
