Your Role Includes:

Evaluating residents and creating individualized treatment plans Supporting residents in improving mobility, balance, strength, and functional independence Collaborating with interdisciplinary teams for resident-centered care Documenting progress according to regulatory and company standards

What We’re Looking For:

Active Wisconsin Physical Therapist license Strong clinical, communication, and documentation skills A passion for working with older adults in assisted living Dependability, adaptability, and a team-oriented mindset
