This is an “as needed” position utilized to cover gaps in coverage including vacations, leaves of absences, and other instances of unfilled shifts. Per diem employees are required to work a minimum of 3 shifts within a 3 month period.
Position: non-exempt, non-union, hourly rate.
JOB FUNCTIONS
- Performs physical therapy evaluations and reassessments per physician referral to assess patients’ physical, functional, social, and emotional needs. Evaluations are accurate, complete, and age appropriate in accordance with departmental policy.
- Interprets evaluation findings and diagnostic information to develop individualized treatment plans and functional goals designed to restore, maintain, and improve patient mobility and functional performance.
- Implements physical therapy interventions based on the established plan of care and modifies treatment as indicated based on patient response, performance, physician input, and updated diagnostic information.
- Assists in discharge planning, including development of home programs, discharge evaluations, and coordination of appropriate follow-up care.
- Provides ongoing education to patients and families regarding treatment plans, goals, safety, and home management strategies.
- Communicates patient status, treatment progress, and care activities with members of the interdisciplinary healthcare team and participates in interdisciplinary meetings, utilization review, and family conferences as appropriate.
- Completes accurate and timely documentation of patient care activities in the medical record, including evaluations, progress notes, reassessments, discharge summaries, and required billing documentation. Ensures patient charges are completed appropriately.
- Utilizes clinical reasoning and professional judgment to prioritize patient care, problem solve clinical issues, and collaborate with physicians and other healthcare professionals when consultation is needed.
- Participates in professional development and departmental activities, including attendance at in-services, providing educational presentations when assigned, participation in community or industrial education programs, and serving as a clinical instructor or job shadow mentor as requested.
- Supports departmental operations and program development, including assisting with new program implementation, maintaining productive use of work time, demonstrating cost-consciousness, and collaborating effectively with hospital staff and departments.
- Complies with all hospital and departmental policies, procedures, and regulatory requirements.
Quality Improvement: Actively participates in the Hospital-wide Quality Improvement Program, actively supports and implements Department-specific Quality improvement initiatives and projects, recommends process improvement as appropriate, reports any quality issues in service delivery and consistently commits to a focus on quality improvement and organizational excellence.
Disaster Management: As an employee of Penobscot Valley Hospital, the position has an inherent role to care for our community members when in need. To this regard, the incumbent will be expected to participate in emergency/disaster preparedness planning and drills as requested. When called upon during a real life disaster/emergency event, the incumbent will be expected to participate in the Hospital’s response to this event, within the scope of professional and personal ability to do so.
Requirements
Graduate of an accredited Physical Therapy Program, diploma, M.S., or B.S. degree required. License to practice or eligible to practice in State of Maine. Valid Maine driver’s license. Previous patient care experience preferred. CPR/Basic Life Support Certification required. Wound Care experience and Certification required.
