This position is responsible evaluating, treating and instructing patients in order to prevent, correct and alleviate impairment and functional limitations resulting from injury or pathology. This position assists patients in reaching their maximum performance potentials while learning to live within their capabilities.
ESSENTIAL FUNCTIONS OF JOB: (other duties may be assigned)
Perform patient evaluations.
Daily, conduct patient evaluations including all pertinent tests and assessments of joint range of motion, strength, sensation, reflexes, transfers, balance, posture and gait.
Complete Homecare evaluations within 24 hours of receipt of the order from the referring home care agency.
Complete and document the evaluation results and appropriate plan of care.
Patient evaluations must be written the day the evaluation is performed.
Contacts the person for verification of specific plan of treatment.
All documentation must be submitted to the agency within 24 hours.
Document therapy plans and progress reports.
The home plan of treatment is updated and reviewed no less than every 60 days.
Daily document therapy plans and progress reports to indicate change in symptoms, measurements and achievement of goals and reports to primary care RN.
Utilize SOAP format in evaluations and daily progress notes.
Perform re-evaluations and write the results every 30 days.
Document patient progress and status to physician when the patient is scheduled to visit the referring physician.
Instructs, Coordinates and communicates physical therapy services with all members of the Interdisciplinary team.
Write discharge summaries and discharge orders and forward to physician within two days of patient discharge.
Documents orders for any changes in care/treatment and visit schedule.
Perform physical therapy patient treatments.
Daily administer therapeutic properties of exercise, heat, cold, electrical stimulation, water, US, massage and traction to patients in an age appropriate and safe manner.
Recognize indications, contraindications and precautions to above modalities.
Perform extensive chart reviews to recognize any contraindications for prescription.
Modify patient position and prescribed program when necessary.
Instruct patients in the correct use of device/brace as necessary.
Daily, measure/assess patient correctly with appropriate device/brace.
Explain purpose and importance of the device in addition to the proper technique and instruction in its use. Obtain patient feedback to measure understanding of the explanation and instruction.
Ensure that physical therapy is delivered in accordance with established standards.
Daily, strictly adhere to American Physical Therapy Association guidelines, department and hospital policies in providing physical therapy to patients.
Evaluate and treat patient only if a referral from a licensed physician is supplied.
Maintain strict patient confidentiality.
Follows all infection control guidelines.
Assist with Performance Improvement Program.
Compile data on current PI project.
Make recommendations for improvement based on data analysis.
Participates in quarterly professional advisory committee meetings and record review.
Schedule patients, prepare daily route sheet, and bi-weekly mileage form
Schedule patient appointments at least 1 day in advance
Mileage form completed and turned in every two weeks
Route sheet turned in daily
Provide in-service training for rehab staff on a yearly basis
Provide correct and appropriate information on in-service topic
Training is to last 30 minutes with proper visual aides
Remain complaint with standard precautions
Demonstrate proper bag technique
Demonstrate proper hand hygiene
Demonstrate proper Respiratory hygiene and cough etiquette
Follow infection prevention policies and procedures.
Qualifications
Education/Training/License/Certification:
Master’s degree or equivalent in a physical therapy field preferred.
Current, valid Commonwealth of Pennsylvania, Department of State Physical Therapy License required.
Valid Cardio-Pulmonary resuscitation certification required.
Valid driver’s license and automobile insurance required.
Work Experience:
One to two years related experience and/or training preferred.
Driving Record:
If deemed a driver in your position, a valid Driver’s license and proof of insurance are required. Motor Vehicle Record checks will be performed upon hire, in addition to annually thereafter.
