Position Summary
The Physical Therapist (PT) provides skilled physical therapy services to patients in their homes in accordance with physician orders, the plan of care, agency policies, and applicable state and federal regulations.
The PT evaluates patient mobility, strength, balance, and functional status, develops and implements therapy interventions, and works collaboratively with the interdisciplinary care team to improve safety, independence, and quality of life.
Reporting Structure
This position reports to the Director of Nursing (DON), Clinical Manager, or designated clinical leadership. The PT functions as part of the interdisciplinary care team and provides supervision to the Physical Therapist Assistant (PTA) and Home Health Aide when required by the patient’s plan of care and agency policy.
Scope of Practice
The Physical Therapist performs services within the scope of Florida PT licensure, Medicare Conditions of Participation, physician orders, and agency policies. The PT is responsible for evaluation, treatment planning, reassessment, discharge planning, and therapy supervision within the home health setting.
Essential Job Functions
1. Patient Evaluation & Care Planning
- Perform initial and ongoing patient evaluations to assess mobility, strength, balance, endurance, and functional limitations
- Complete comprehensive assessments and participate in OASIS-related coordination as applicable
- Develop, implement, and update individualized physical therapy treatment plans
- Ensure physician orders areappropriateand aligned with the patient’s therapy needs
2. Skilled Therapy Services
- Provide skilled physical therapy interventions in accordance with physician orders and established care plans
- Guide patients through therapeutic exercises, gait training, transfer training, balance activities, and mobility restoration
- Instruct patients in the safe use of assistive devices including walkers, canes, wheelchairs, and other mobility equipment
- Support fall prevention, pain management, and restoration of safe functional movement in the home setting
3. Patient & Family Education
- Teach and counsel patients and caregivers regarding therapy goals, home safety, fall prevention, mobility strategies, and self-care techniques
- Evaluate the home environment and identify safety modifications or community resources needed to support care
- Coordinate outpatient services when procedures cannot be safely performed in the home
4. Documentation & Compliance
- Complete timely and accurate clinical documentation, progress notes, reassessments, and discharge summaries
- Evaluate effectiveness of therapy interventions and update the plan of care as needed
- Maintain compliance with Medicare requirements, state regulations, and agency policies
5. Team Collaboration & Supervision
- Communicate regularly with physicians, nurses, and interdisciplinary team members regarding patient progress and needs
- Supervise the Physical Therapist Assistant (PTA) in accordance with agency policy and regulatory requirements
- Supervise the Home Health Aide when Physical Therapy is the only skilled discipline involved in patient care
- Participate in care conferences, discharge planning, staff meetings, and required in-services
Qualifications
- Graduate of an accredited Physical Therapy program required. Current state Physical Therapist (PT) license in good standing required. Minimum of one (1) year of experience in physical therapy required; home health, acute care, or rehabilitation experience preferred.
- Current CPR certification required
- Valid driver’s license, reliable transportation, and auto insurance
- Ability to pass background screening and all compliance requirements

Physical Requirements
- Frequent travel to patient homes within assigned territory
- Ability to stand, walk, bend, lift, and assist patients as needed
- Ability to perform therapy assessments and interventions in home environments
- Ability to use electronic documentation systems and therapy equipment
- Ability to communicate effectively with patients, families, and care team members
Work is performed in patient homes and community settings with exposure to variable environmental conditions, including pets, smoke, infectious diseases, and weather-related travel conditions.
