Greenfield Rehabilitation Agency is seeking a Passionate PRN Physical Therapist (PT) to join our team at The Veterans Home in King, WI. Physical Therapist Responsibilities
Conduct initial and ongoing evaluations of residents’ strength, mobility, balance, and endurance
Identify barriers to independence and functional movement
Develop individualized treatment plans to improve mobility, strength, and daily function
Provide interventions for recovery from injuries, surgeries, or illnesses
Assist residents in performing activities of daily living (walking, dressing, transferring)
Implement exercises to improve coordination, flexibility, and overall physical function
Educate residents and staff on safe mobility and fall prevention
Recommend and train residents on the use of assistive devices
Collaborate with a multidisciplinary team including nursing, OT, and SLP
Maintain accurate documentation to ensure compliance with healthcare regulations

Qualifications
Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
License as required by the State of Wisconsin to practice as a Physical Therapist.
