Physical Therapist – Home Health

Position Summary
As a Physical Therapist, you will assess, plan, and deliver individualized therapy programs designed to improve mobility, relieve pain, restore function, and enhance overall patient well-being. You’ll work closely with patients, families, and interdisciplinary care teams to provide exceptional home health care services.
What You’ll Do
- Perform and document comprehensive patient evaluations and establish treatment plans
- Develop and implement individualized therapy programs to restore physical function and mobility
- Monitor patient progress and modify treatment plans to maximize outcomes
- Educate patients and caregivers on exercises, safety, and continued care strategies
- Maintain timely and accurate clinical documentation
- Collaborate with physicians, nurses, and healthcare professionals to ensure continuity of care
- Follow infection control and universal precaution guidelines
- Support the agency’s mission, values, and patient-centered care philosophy
Qualifications
- Graduate of an accredited Physical Therapy program
- Current Physical Therapy license
- CPR Certification
- Valid driver’s license and auto insurance
- Home Health experience preferred
- HomeCare HomeBase experience preferred
Physical Requirements
- Ability to stand, walk, bend, kneel, squat, and move throughout the day
- Ability to lift, carry, push, or pull up to 100 lbs.
