We’re looking for an experienced Physical Therapist to join our Home Health team in a full-time capacity. This opportunity is ideal for a PT who values autonomy, relationship-building, and delivering meaningful care where patients are most comfortable — at home.

What You’ll Do
- Provide skilled physical therapy services to home health patients across the Omaha metro area
- Develop individualized treatment plans focused on mobility, strength, safety, and independence
- Partner closely with nurses, providers, caregivers, and interdisciplinary teams to support patient goals
- Complete timely documentation and maintain compliance with all regulatory requirements
- Educate patients and families on exercises, safety, and continued care plans
- Build trusted relationships with patients while delivering compassionate, high-quality care
What We’re Looking For
- Licensed Physical Therapist in the state of Nebraska (or eligible for licensure)
- Previous home health experience strongly preferred
- Minimum of 1+ years of PT experience required
- Strong clinical judgment, communication, and organizational skills
- Ability to work independently while staying connected with the care team
- Reliable transportation and valid driver’s license
