POSITION SUMMARY: Physical Therapists are responsible for overseeing, planning, and administering physical therapy evaluations and treatments to Rocky Mountain Health Care Services (RMHCS) program participants. The position blends outpatient, home health, and geriatric (55+) focused therapy to restore function and prevent disability. This position also acts as a member of the interdisciplinary team (IDT) as needed.

RESPONSIBILITIES AND DUTIES:
ESSENTIAL JOB FUNCTIONS:
Serves as a required member of the Interdisciplinary Team and attends IDT meetings daily, or as required
Performs required assessments. Plan and prepare physical therapy treatment plans based on thorough evaluation and completes/updates corresponding interdisciplinary plans of care with physical therapy interventions and goals
Tests and measures participant strength, motor function, sensory perception, functional capacity, and respiratory and circulatory efficiency, and records findings to develop or revise physical therapy treatment plans
Administers treatments involving application of physical agents
Evaluates effects of physical therapy treatment at various stages and adjusts treatment plans to achieve maximum benefit
Evaluates, fits, and adjusts prosthetic and orthotic devices and recommends modification to Orthotist
Oversees all aspects of the physical therapy treatment plan, including the delegation of therapy to Physical Therapist Assistants (PTA), according to complexity of participant’s needs and nature of required physical therapy interventions
Supervises and directs Physical Therapist Assistants, rehabilitation technicians, aides, and students
Trains family members, caregivers, and other staff members in methods to enhance mobility, safety and functional independence of participants
Evaluates for and provides training to participants and caregivers for durable medical equipment
Fulfills company documentation requirements
Other duties as requested/assigned
ADDITIONAL ESSENTIAL FUNCTIONS – SPECIALIZED IDT POSITION:
Acts as a member of a specialized interdisciplinary team that supports participants residing in contracted long-term care facilities
Physical therapy in long-term care helps residents keep their independence, lower chronic pain, and move safely
Care plans focus on slowing physical decline rather than intense short-term recovery; therapists assess each person to prevent falls and offload burden of care
Expertise in ongoing communication with the leadership of the Long Term Care (LTC) facilities to include, but not limited to, Directors of Nursing, and Rehabilitation Directors, as well as skilled rounding, staff education, and training are essential functions of the role
Qualifications
Completion of a Doctor of Physical Therapy or 4-year program from an accredited school is required.
2 years of relevant work experience preferred
2 years of experience working with an adult or geriatric population preferred
Must have working knowledge of computers and Microsoft Office applications
Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public
