AlwaysNear Home Care LLC is looking for a dedicated and compassionate Physical Therapist (PRN) to join our growing team in Atlanta, GA. Responsibilities:
- Conduct comprehensive physical therapy evaluations and assessments for home care patients
- Develop and implement individualized treatment plans to restore function and improve mobility
- Provide therapeutic interventions including therapeutic exercise, manual therapy, and neuromuscular re-education
- Monitor and document patient progress, adjusting care plans as clinically appropriate
- Educate patients and caregivers on home exercise programs and injury prevention strategies
- Collaborate with interdisciplinary home health team members to ensure coordinated patient care
- Maintain accurate and timely clinical documentation in compliance with regulatory standards
Requirements:
- Valid Georgia Physical Therapist (PT) license in good standing
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Home health or outpatient physical therapy experience preferred
- Excellent communication, interpersonal, and patient education skills
- Reliable transportation and valid driver's license for home visit travel throughout Atlanta, GA
- Ability to work independently and manage a flexible PRN schedule
