Aspirus At Home is seeking a Physical Therapist to join our field team to cover in both Wisconsin Rapids and Stevens Point, WI!
The Physical Therapist provides physical therapy to adults and geriatric patients. Evaluates, establishes treatment plans, and goals of treatment, and provides for and/or supervises the administration of treatment modalities/procedures done to meet the needs of the patient as it pertains to the age and diagnosis of the patient. Incorporates patient and family education. Provides documentation for all the above.
This position offers a $20,000 sign on bonus (to those that are qualified), mentorship, hourly wage, mileage reimbursement, and flexibility most people crave!
HOURS: Full-Time or 1.0 FTE, 80 hours every two weeks. Day shift, with weekend rotation
Responsibilities:
- Evaluate patients and establish treatment plans
- Provide physical therapy to adults and geriatric patients
- Supervise administration of treatment modalities/procedures
- Incorporate patient and family education
- Provide documentation
Requirements:
- Knowledge of physical therapy standards and practices normally acquired through completion of a minimum of a bachelor's degree in physical therapy from a school of Physical Therapy accredited by the American Physical Therapy Association (APTA)
- Current Wisconsin and/or Michigan licensure to practice Physical Therapy required
- Current BLS certification or obtained within 90 days of hire
- Possesses and maintains a valid Driver's License required
- Possesses automobile liability insurance with $100,000/$300,000 coverage required, or $300,000 combined single-limit liability
