SummitWest Care is seeking a compassionate Physical Therapist to provide one-on-one care throughout Mesa County.

Responsibilities
- Evaluate patients and develop individualized, person-centered care plans
- Provide skilled physical therapy services in patient homes
- Improve patient mobility, strength, balance, and functional independence
- Educate patients and caregivers on treatment goals and home exercise programs
- Monitor patient progress and modify treatment plans to support individual goals
- Complete accurate and timely clinical documentation
- Collaborate with nurses, therapists, physicians, caregivers, and other members of the care team to achieve the best possible outcomes
Qualifications
- Current Colorado Physical Therapist (PT) license in good standing
- Graduate of an accredited Physical Therapy program
- Valid driver's license, reliable transportation, and auto insurance
- Strong clinical assessment and treatment skills
- Excellent communication and documentation abilities
- Ability to work independently while collaborating effectively with an interdisciplinary team
- Ability to adapt professionally in a home-based care environment
