Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate and dedicated Physical Therapist for a PRN field position providing exceptional in-home therapy services throughout the Fort Worth area.

Responsibilities
- Evaluate patients and provide skilled physical therapy services according to physician orders and individualized treatment plans
- Implement therapeutic exercises and interventions to improve mobility, strength, balance, pain control, and functional independence
- Assess patients’ home environments and recommend equipment and safety modifications to support successful recovery
- Educate patients and caregivers on home exercise programs, mobility techniques, and fall-prevention strategies
- Complete timely and accurate clinical documentation in the electronic medical record
- Collaborate with the interdisciplinary team to coordinate patient care and communicate patient progress
- Maintain compliance with agency policies, Medicare regulations, and all applicable state and federal standards
- Travel to patients throughout Fort Worth and the surrounding area
Qualifications
- Current Texas Physical Therapist license in good standing
- Graduate of an accredited physical therapy program
- Minimum of one year of physical therapy experience
- Home health experience preferred
- Current BLS certification
- Valid Texas driver’s license
- Reliable transportation and current automobile insurance
- Strong organizational, communication, and clinical skills
- Experience with electronic medical records preferred
